We have an open list and welcome requests for registration from patients living in or moving to the practice area, which covers: Tarporley, Eaton, Utkinton, Little Budworth, Alpraham, Bunbury, Huxley, Tarvin, Kelsall, Ashton Hayes, Mouldsworth, Delamere and Oakmere.

Please complete ALL the forms below (either PDF or Microsoft Word version), which are also available from reception, including your NHS number and details of your previous doctor.  A registration form will need to be completed for each member of your family that wishes to register with the practice.

  • Ukrainian Rapid Health Risk Assessment: PDF or Microsoft Word – please complete this form if you are from Ukraine with as much information as you can. You do not need to complete the New Patient Health Check Form above but please complete the other forms in the list above.

Please complete all of the sections on the forms including your NHS number and details of your previous doctor.  A registration form will need to be completed for each member of your family that wishes to register with the practice.

ID and proof of address

You will need to provide proof of identification and address when handing in your registration forms. Please provide one of the following documents from each of the lists:

Proof of Identity:

Passport, driving licence, birth certificate, marriage certificate, medical card.

Proof of Address:

Utility bill, bank/building society statement, payslip (with address), external correspondence (solicitors/estate agent, showing name and address).

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access, please collect a proxy access form from reception from 9am to 5pm.

Linked profiles in your NHS account

 Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
