On-Line Access To View Your Medical Records

In line with government guidelines patients age 18 and over can now have on-line access to detailed coded records as part of Patient Access, as well as making routine appointments with their Doctor and ordering repeat prescriptions on-line.  If you are not registered for Patient Access please click here to register and follow the instructions.  You will need our postcode which is CW6 0BE.  Please select Drs Adey & Dancy from the pick list.

This will include:

  • Results e.g. blood pressure, blood tests, records of X-rays
  • Vaccinations and immunisations
  • Medications
  • Problems/Diagnoses
  • Allergies, active problems
  • Codes showing referrals made or letters received (no attachments)
  • Other codes

Being able to see your record online might help you to manage your medical conditions. It also means that you can access it from anywhere in the world should you require medical treatment on holiday.

Those patients who wish to use Patient Access to view their coded medical record will need to complete an application form. The form is downloadable here.  Please note you will need to be registered and accepted for Patient Access before you apply to view your medical record.
