Covid Pass Certificates

Please note we are unable to issue Covid Pass Certificates at the practice.

Proof of your vaccination status will also be available on the NHS app (for people aged 13 and over).  To download this to your device:

To view your Covid vaccinations on the NHS App: from the home page go to “Your health” then from here you can view your NHS Covid Pass and check your vaccine record.

Alternatively you can:

  • Call the NHS helpline on 119 and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine.  The letters are valid for 30 days and you should receive it within 7 working days of the date you ask for it. Do not call if you’re travelling in more than 4 weeks’ time.
  • Use the NHS on-line Covid Pass Service (age 12 and over) to download and print an NHS Covid Pass . You’ll be asked to create an NHS login if you do not have one already.

The NHS COVID Pass letter will be posted to the address held on your NHS record (the address held by your GP) and will show all the vaccinations you have received, including any boosters.

The NHS COVID Pass letter service is for international vaccination status only and will not show recovery or negative test results.

Please note we are unable to provide a Covid-19 Recovery Certificate. This is not an NHS service. This can be provided by a private company for a fee.


Covid Pass for children aged 5 and over

Fully vaccinated children aged 5 and over can now get an NHS COVID Pass letter for travel. We are unable to provide this for you so please allow plenty of time in requesting this if you are travelling abroad.

If you need a Covid Pass for fully vaccinated children aged 5 and over:

  • Call the NHS 119 telephone service to request an NHS COVID Pass travel letter. This must be at least 5 days after your child has completed the  course of the vaccine.  The letters are valid for 30 days and you should receive it within 7 working days of the date you ask for it. Do not call if you’re travelling in more than 4 weeks’ time.
  • Go online at Get your NHS COVID pass letter (age 12 and over) to download and print an NHS Covid Pass . You’ll be asked to create an NHS login if you do not have one already.
  • From 3 February 2022 children aged 13 and over can register to use the NHS app and set up their own account (using their own e-mail address) in order to access their digital NHS Covid Pass.

A person with legal responsibility for the child can request the letter on their behalf.

The NHS COVID Pass letter will be posted to the address held on the child’s NHS record (the address held by their GP) and will show all the vaccinations they have received, including any boosters.

The NHS COVID Pass letter service is for international vaccination status only and will not show recovery or negative test results.

Please be aware that we do not register children under the age of 18 for proxy access on the NHS app, therefore, we are unable to set up a linked profile and you won’t be able to have a digital copy of your child’s Covid-19 vaccination if they are under the age of 13.  If you require proof of your child’s Covid-19 vaccination please request a paper copy as above.
